Our mission is to improve the health and wellbeing of regional and rural Australians, by empowering people living with a chronic illness to live full and meaningful lives. 

Our key objectives are:

CHRONIC DISEASE MANAGEMENT IN THE COMMUNITY

To improve mental, social and physical health outcomes for people living with chronic illness through the provision of integrated multidisciplinary, preventive and community-based health and wellbeing programs and services.

RESEARCH & EDUCATION

To be a centre of excellence and a learning centre for students and educators to undertake research and to disseminate community-driven research.

HEALTH PROMOTION & COMMUNITY CONNECTION

To connect people with each other, community and wellbeing services, develop life skills and promote healthy behaviours.

COOPERATIVE HEALTHCARE  

To establish a sustainable, innovative and well-governed healthcare cooperative which integrates its services with primary care.

CONSUMER ENGAGEMENT & ADVOCACY

To ensure consumers with lived experience and the community are engaged in the ongoing co-design and governance of the Circle Foundation Cooperative.
